CHAMP California Accountable Communities for Health (ACH) Project, led by Dr. Diane Rittenhouse, has now made available its ACH Data-Sharing Toolkit, available for free download. This toolkit, sponsored by the California Health and Human Services Agency, provides a resource to assist communities in California and beyond in working to share data across sectors for health improvement. The California Accountable Communities for Health Initiative, which aims to improve the health of each community by linking health care and community prevention activities, requires complex and cross-sectional data and information among community partners, and the current data-sharing capacity can be highly variable across communities. The ACH Toolkit responds to the need for a resource to facilitate ACH collaboratives in California, providing the steps and processes required to engage in data-sharing across sectors.
